HOW TO PREPARE WYETH ASCENDA

WARNING: Unboiled water or incorrect dilution can make your child ill. Incorrect storage, handling, preparation, and feeding can potentially lead to adverse effects on the health of your child.

1. Wash your hands and utensils before preparation.

2. Boil water and allow to cool.

3. Strictly follow the feeding table. First, pour water and then add powder. Stir until the powder is fully dissolved. Check temperature before feeding.

4. Make sure the scoop is dry before introducing back into the bag.

RECOMMENDED SERVING

One scoop = 6 grams (approx.). Mix 1 scoop of ASCENDA to approx—30 mL (1fl. oz.) of water.

INTENDED USE / REMINDERS:

- Prepare only one serving at a time. Feed immediately and follow the instructions exactly. Do not keep unfinished serving, discard the contents. Always hold child while feeding. Leaving the child unattended may cause choking.

- Using more or less powder than indicated could either lead to dehydration or deprive your child of proper nutrition. Do not change proportions without medical advice.

- Expiration date and lot identification code are printed on the bottom of the can/pack. Use before the expiration date.

- Avoid prolonged storage at excessive temperatures.

- Fill is by weight, not volume. Some settling of the powder may occur.

- Please seek the advice of your doctor or dietitian to help you assess your child's growth concern and develop an action plan if needed. Various factors can impact children's growth and a careful assessment of their growth pattern over time is highly recommended.
